Required Registration Documents

The documents must be completed and signed before camper registration is completed:

• A participation waiver is required for acceptance to the camp. Each camper must complete and return a waiver prior to the first day of camp. The waiver must be signed by the parent or guardian of the camper:

• If anyone other than the listed parents/guardians intend to pickup the camper, a completed and signed sign-out sheet must be filed with the camp office:
